A prescription is required. We are not physicians, so we are unable to diagnose your symptoms. However, with a diagnosis from your physician, we can custom design the most appropriate foot orthotic based on the diagnosis, physical exam, and activities of daily living.

If you have a valid prescription and insurance coverage for custom foot orthotics, our office will bill your insurance. The patient would need to make this selection prior to the fabrication of the device to ensure appropriate documentation is obtained. Only applicable copays and co-insurances will be collected if insurance is being billed.

This promotion can be utilized if you do not have coverage for custom foot orthotics or would like to purchase additional pairs that are not covered by your insurance.
